Suppliers must:

• Understand and meet our business and clinical requirements.
• Be responsive to evolving healthcare needs.
• Deliver the highest quality products and services at competitive cost.
• Conduct all business ethically and in compliance with hospital, local, and state standards.

Supplier Application & Review

All supplier submissions will be reviewed by Anaheim Community Hospital’s Procurement Team.
Suppliers will be contacted directly if their application is approved or if additional information is required.

• W-9 Form
