Release of Patient Information
Anaheim Community Hospital (ACH) is required by law to maintain the privacy of your health information, to provide you with a notice of our legal duties and privacy practices, and to follow the information practices that are described in the Notice of Privacy Practices.
You have the right to receive a copy of your health information that we maintain, with some limited exceptions. You have the right to receive a copy of your health information in one of the following formats: email, fax, or paper via mail. You have the right to request that your health information be sent to any person or entity.
Medical records are maintained by the hospital for the time period required by state law, and some medical records may not be available.
Processing time for copies of medical records with valid authorization
Please allow sufficient time to process a medical record request. Turnaround time varies according to request type and state law:
- California: up to 15 business days
Fees for copies of medical records
Continuation of care: Medical records sent directly to providers or other healthcare facilities: No Fee
Patient/patient representative: A reasonable fee may be charged based on the requested medical record volume. The fees are based on State and Federal Regulations.
Other requester types: Fees are based on State and Federal Regulations
Payment for copies of medical records
Follow the payment instructions on the medical records invoice you receive. Payments for medical records may be made via check or credit card.
How To Obtain a Copy of Medical Records
- Download and print our Release of Information Form – Anaheim Community Hospital.
- Complete, sign, and date the form. To verify your identification and validate your authorization, you are required to include a legible copy of a valid photo identification (i.e., a driver’s license, a military ID, or a state ID).
- Email, fax, mail, or deliver your paperwork in person. Please see further instructions below.

Continuation of care requests
For immediate continuity of care requests, you or your healthcare providers may request that records be sent directly to their office.
The physician’s office must send a written request on business letterhead to (714)699-5591 (fax). Please indicate the patient’s name, date of birth, visit/admission, and requested information. Please indicate “STAT” at the top of the request. For immediate assistance, call (714) 243-9000.
Copy service/subpoena
Requests must be hand-delivered to personnel in our Health Information Management Department (with the exception of criminal subpoenas, which are accepted via certified mail or hand service). Fees for services will be billed.
Important notice
Incomplete requests are considered invalid and will be returned for additional information.
Patient representatives may need to provide supporting documentation to fulfill the medical records request, e.g., Durable Power of Attorney, Advance Directive, guardianship, or conservator forms.
